Pyrazinamide was first synthesized in 1936, but its anti-TB effects were discovered only in 1952. In the 1970s, it was first used as a second-line treatment against drug-resistant TB but would eventually become a first-line treatment against active TB.

Pyrazinamide's mechanism of action is not well understood, it was thought to inhibit fatty-acid synthase but that theory has been discontinued. The leading theory now is that it interferes with energy production as Pyrazinoic acid, its active moiety, accumulates within the bacteria.

While joint pain is the most common side effect. Pyrazinamide is more well-known to be hepatotoxic but other effects such as nausea and vomiting, anorexia, malaise, and fever have been documented.
