Actions in life would shape your own personal hell, which awaits you in death. Cunning demonologists could curate their hells intentionally, using rituals to draw power from them and summon their demon as a minion though it’s a double-edged sword, as you are empowering your own torment after death

That is some excellent stuff right there. I had a similar idea of holy warriors being accompanied by guardian angels, however should the warrior fall, the angel falls with them and turns into a demon. I think it could be used as a set up for an antagonist or show the price of failure to a character

That's pretty thematic and strong. Perhaps the angel represents the ideal you aspire while the demon contrasts by accepting/representing you for who you truly are (the darker aspects). perhaps a cool theme to play around with.
