This was a paper-based review, with no evidence from witnesses. Conclusions are provisional, with Pughsley given opportunity to update after further RMP witness evidence.
Topic 1: Inception of Op Northmoor up until July 2016.
- 'I have found that the strategies adopted and implemented by Gold and the SIO...did not progress the investigation at the pace, breadth, or depth required for a category A investigation'
- Delay was not the fault of the RMP.
- But, suprising lack of Gold Command meetings between 2014-16 and few records explaining Gold decision making during this period. 'Gold minutes were scant'.
- Re: two groups of independent advisors expressed 'consistent and growing concern', but 'recorded concerns were largely ignored'.
