Do you have a link to an informed consent about the study? The link you gave goes immediately to the first page of your survey and I suspect that practitioners will want more info about who you are and how their data is being kept confidential before answering even question #1 about their practice.

The first link is for a screening questionnaire, not the study itself. Once folks are screened to be potential participants they will receive the Participant Information, Consent and around 1 survey. If folks are concerned or would like more information I would welcome a DM or email.

I appreciate the concern in the current climate. I am more than happy to provide the Participant Information prior to the screening questionnaire on request. On reflection, I should have added it to the questionnaire.
