Reskinning a giant ape into a giant sea ape. Giving it 40 ft swim speed + the ability to breath under water? Should I scale back base health to account for these buffs, or are they tiny enough to ignore?

I'd say only scale it back if the party is at a disadvantage in the water. But HP is one of those things you can change on the fly if the encounter is taking too long.
Then I'd recommend not nerfing the hp. My personal rule is to only nerf the HP when I make it harder for them to damage it, higher AC, extra resistances, etc.
