In North Dakota, about 85% of women report being asked about intimate partner violence (IPV) during pregnancy, and this drops to 76% postpartum. Providers I work with know this is an important conversation to have with patients, but limited time and lack of IPV resources is inhibiting.

Also, rural residents in ND are more likely to be screened for IPV both during and after pregnancy than urban residents. This disparity appeared to worsen during the pandemic (not sure why - overworked providers?) and haven’t seen enough data to know if this rebounded. Still writing the paper.

As a note, we have examined role of IPV in disparities in prenatal care access and breastfeeding in ND - but disparities in both outcomes remained after accounting for IPV.

Something innovative is needed to address IPV in pregnancy/postpartum. We recently put a survey out for providers regarding quality issues (like screening) and provider bias, hoping this provides a piece of the puzzle.

The question of what the best way to ask about IPV remains. Then we we need to build coalitions to better link IPV survivors with resources. However, whatever the answer is, those of us working on this issue need to better engage IPV survivors, families and communities.

We also can’t forget the importance of doing this work in a culturally safe way. In ND this not only means our Indigenous communities, but our rural communities and our fast growing New American populations. Different people will need to address IPV in different ways!
