There used to be a random event where your teleport failed, and left you in an abyss like room until pressed levers or something, if I remember correctly
So while I'm not saying it's canon, it's definitely not far off

I low-key loved that random event. I get that it's anti-efficiency but just made the world feel a lil more chaotic.
I'd be ok with all the old random events returning personally.
