I worked with claims for 20+ years. A majority of the denials I worked on got reversed by simply asking the provider to send in the clinical notes that should have accompanied the original request. So is it okay to gun down providers in cold blood too, for failing to handle a simple admin task?

P.S. I've seen more receipts than they have, but I also comply with a little law called HIPAA.
