An occupational therapist can come to your house to evaluate it and recommend household equipment, furniture, layouts.
A speech therapist can help with cognitive/memory/speech issues.
A physical therapist can recommend and fit mobility equipment.
There are disability catalogs. That's right, entire product lines of adaptive equipment for every room in your house and many activities.
Also, specialty hospitals are where it's at. If you have access, of course.
