Alrighty, I got this working well with profiles - so now I have a profile for "Database work" and one that is my default. They don't share settings or keyboard shortcuts, so I was able to cusotmise the Databases one nicely. The actual extension still has room for improvements but it's getting there

Did you know we now have first class built in support for markdown in Visual Studio 2022?! You're also supposed to be able to create a new markdown file from template via Add New File, although that's not working for me yet (odd, since it's in my What's New!) Still, love seeing this advancement!
