For Gray in the Witcher TTRPG: She has several, but I'll say the fight with her cursed mentor. She could have saved him by breaking the curse, but she instead returned his lessons of ruthlessness and expediency to him. She knows she didn't do the "right" thing, and that will always haunt her.

For Fire in Cyberpunk: Definitely the fight with Ruthven, the antagonist from a Night at the Opera. One of the crew had just gotten chewed up by a turret, and it was a challenging fight in general, with high stakes. The encounter reminded Fire that she's not invincible. Image credits in alt text.
