Is the goal some kind of a “good enough” gvt? Say maybe an airline safety standard like Aeroflot? Maybe a 5% chance of a crash? Maybe Medicaid only pays for the lowest tier of care, aspirin and stitches and too bad for you if you need more. If Poors die it saves money.
