There are remarkable things medicine can do. And, we also need services to be caring & containing for people who are in a vulnerable place. We need both. But with services so stretched, the medical model gets more foregrounded as essential, & the human side gets reduced to being less important.

It's a constant source of anxiety in the NHS to have to cut corners all the time- to know that if anyone scrutinised your work they'd be able to find faults. You just hope nothing bad happens & that you've done enough. It's not fair that we have to work in these conditions, it's making us ill.

Lots of people given a BPD/EUPD are offered DBT. No one referred to DBT is told that the creator of the therapy advises people “for gods say don’t tell anyone” about their diagnosis because of the stigma and prejudice that will be enacted. Why don’t we tell people that? youtu.be/G08IYl2DAMg?...

It's frustrating in NHS when you need input from other people to move things fwds, but what's a high priority to me isnt a high priority for the other people. It's nobody's fault, it's just people are trying to juggle so much. And my sense is this is happening all the time, all through the system.

I enjoyed presenting some of my PhD research in Glasgow today, at an NCISH toolkit event with mental health nurses from across Scotland. Recognition of failings and enthusiasm for change.

This from 2003 is an excellent analysis of mental health nursing in acute inpatient settings, which details nurse experiences, and calls for an abolition of these settings in their current form. Still pertinent over 20 years later.

I've been re-reading this on Clozapine for 'BPD' Fascinating bit that says NHS clinicians were uncomfortable prescribing clozapine. It was only on the independent "specialist" units that this clear breach of NICE guidance was embraced. pmc.ncbi.nlm.nih.gov/articles/PMC...

When we're in the middle of fire fighting an understandable reaction would be to build fire stations on every street corner. But surely preventative/environmental measures remain the absolute key thing, with well resourced emergency services available when needed. And the same applies in healthcare

I don't post much about #SpacesForListening but it's on my mind just how grateful people are for coming along. It's like it's out of proportion. It's literally just a 50min zoom call where we listen to each other. I guess it shows just how absent this kind of space is in our normal ways of working.

I meet people in significant psychological distress & yes it can be helpful to talk to someone. But sometimes what'd be more helpful would be someone to tidy up the house, to make a healthy dinner, to help out with the kids- practical help. Money. A break. So many people living in chronic stress.

Something i'd love to see emerging from #ChangeNHS is that all NHS people have access to regular reflective space. We KNOW this is a core part of decent care & it'd help with job retention if we all had a space to think & feel supported. I'd be genuinely excited to see this as a top down commitment.

Excellent piece outlining the benefits and pitfalls of diagnosing yourself with a mental disorder, using an analysis of Reddit posts. Generally, self dx was seen as limited, often negative, and requiring some form of clinician input to be meaningful. journals.sagepub.com/doi/10.1177/...
