Ok. Crazy but it works ICU pearl. I do SBT on morbidly obese (BMI>40) pts on the high peep they were on for their habitus. If they pass, I extubate. Almost never need bipap after. Don’t know why spontaneous breaths better than SBT on peep of 5. Any ideas?? #pulmsky #ccm
