I'll just spitball a few. Xenoblade 3 chapter 5 is obvious. Reverie's ending is masterful, in so many ways too. Ever17's Tsugumi reunion scene is especially visceral, the backstory and CGs together. Triangle Strategy's Frederica ending is simple, but still powerful.

Xenoblade 3 End of chapter 5, beginning of chapter 6 is for sure up there, for a variety of objective and personal reasons. Outside of that, the opening of Saber route Feb 13 is one of the most personable things I've ever seen. The line "I...what, just tell her she did a good job?" Still breaks me.
