excellent piece by @mirandayaver.bsky.social in @statnews.com on how health insurers use administrative burdens to "ration by inconvenience" require prior auth, deny claims -- "it's ok because they can appeal" but the appeal process is daunting, confusing, takes a ton of time and effort etc

not only are insurers effectively not liable for administrative burdens they place on patients an physicians as obstacles to care, but they are also incentivized by short-term savings even if it has long-term higher spending because they know people change insurance with jobs, age into medicare etc

Good news but need to see the HR and subgroups here, wish they hadn’t teased the “1-year” improvement in median OS. The interim OS HR was 0.8 and 5% diff at 2 years so curve probably very flat around the median meaning median diff can be huge with less impressive HR. #LCSM

Starting TODAY, a key provision of the Inflation Reduction Act goes into effect: Limiting annual out-of-pocket spending on prescription drugs to $2,000 for Medicare beneficiaries. 19 million people are expected to save an average of $400 each.
