Throughly enjoyed Alex Garland’s #CivilWar (& certainly more than critics and online reviewers) Kirsten Dunst, Jessie Plemons in novelty sunglasses being evil, two Suicide records, and a real Last Days of Hitler vibe at the end. Good stuff

Looking like an Egyptian relic #Caturday

Me, listening to your presentation on the post take ward round

#Cricket on the radio and #mlb spring training overnight Summer is coming

Looking for a new podcast Suggestions please? (Things I’ve liked before: Joe Pera, West Cork, In Our Time, Hysterical)

#NephMadness still days away and yet intimidation of the Blue Ribbon Panel has already begun #HighStakes

Great stuff here Press: www.theguardian.com/society/2025... Paper: www.thelancet.com/journals/lan... Seems initial strides in #GeneTherapy are in tissues where direct injection is possible ie retina as above and cochlea such as: investor.regeneron.com/news-release... Hopefully kidney soon!

One of my favourite ways to waste time is to go to the Stanford Encyclopedia of Philosophy and go to > Browse > Random Entry No, _you're_ pretentious plato.stanford.edu/index.html

Was quite proud of this but then @bsky.app is a much tougher mistress than the dopamine flood of twitter

Musk is a venture capitalist, not a scientist. VCs invest in young companies solving big problems, grow them, & exit with huge profits. The US won’t see growth, yet Musk will profit. America didn’t ask for his investment, and will get despotism as their share of the deal. PS The book below is good

I think all the points in this apply to Drs too www.theguardian.com/society/2025...

“Hitler systematically disabled and then dismantled his country’s democratic structures and processes in less than two months’ time—specifically, one month, three weeks, two days, eight hours, and 40 minutes,” Timothy W. Ryback writes in a step-by-step account:

What’s the best #cowboy TV series? Deadwood? Godless? Yellowstone? Justified (does that even count)? The English? I love all these btw

Unbearably awful www.thefederation.uk/news/mrcpuk-...

RCP elections coming up Recently, the College has been aimless & adrift; I resigned from the editorial board of one of its journals in protest. I'm confident that @drlkvaughan.bsky.social for CVP and @drasifqasim.bsky.social for PRCP will right its course #TimeForChange www.rcp.ac.uk/news-and-med...

We are living in the 1930/40s, so here’s Lord Haw Haw

I have a feeling of deja vu. The RCP actually conducted a review of training not even 18 months ago. Let's have a little look at it, shall we? 🧵 rcp.ac.uk/policy-and-c... 1/

I wrote a thread about #NHS productivity from my pov of 20yrs clinical experience & no policy knowledge I linked to stuff from the ONS & FT, but when it came down to it: - clinicians are the productivity "talent" - Tom Cruise never had to be the leading man, key grip, & gaffer on any of his movies

Doctors who made it social media famous during covid saying they’re still on X to “try and debate and change hearts and minds” are, at this point, like addicts saying they could quit tomorrow

So is the forthcoming decline of America going to be most similar to an ancient civilisation vanishing like the Mayans or Easter Island, or the regression of an empire like the Austro-Hungarians or the USSR? Either way it’s going to be fascinating to see what happens

- Single kidney post nephrectomy with halving of eGFR down to low 20s - Minimal albuminuria (3mg/mmol) - Dilated cardiomyopathy Immediate RAASi/SGLT2i/MRA or wait for function to "stabilise" (whatever that may mean)? #NephSky

Pet peeve (that on xshitter would have provoked a pile on): Resident doctors should not be referring to themselves in notes by their initials ie WR JS where it's IMT1 John Smith doing the ward round I certainly never refer to myself in the notes as TO or equivalent

As someone about to send another word doc to a writing group and thus start another round of numerous other track changes versions, what was the conclusion of the discussion you started on this recently @doced.bsky.social ?

Just watched American Hustle. Nice movie So apart from The Godfather, its own part 2, Casino & Goodfellas what are the best #mafia movies? I like Carlito's Way, The Untouchables (cos they came out when I was impressionably young) & Gomorrah You?

I am currently reviewing a paper which makes some extravagant and illogical claims about digital twins. However, I don't want to dismiss the paper because of this. Is there a serious critical analysis of the potential of DTs that can be recommended?

I've not read this article yet, but I frequently maintain that The Breakfast Club is probably the single worst cultural artefact widely held up as a touchstone.

Middle aged life is just thinking "Oh yes, I'd quite like to see that movie at the cinema" and then realising going to the cinema is a time suck of unimaginable decadence

Happy Valentine's to everyone Except people who make AKI referrals quoting eGFRs (Yes, I have had no new material since about 2017)

America is teetering on the brink of something no one can name, predict or quantify If you don't know about the Samoa reference here , do read: en.wikipedia.org/wiki/2019_Sa...

A problem I foresee with the use of LLMs & AI scribes in medicine (yes, I realise it's a frivolous one) is how verbose and therefore boring and heart sinky they are to read. No one wants to wade through pointless adjectives and adverbs and clunky sentences to get to the referral reason/diagnosis etc

📢New publication!📢 1st journal article from my NIHR Advanced Fellowship: protocol for a systematic review on the prevalence of #kidneydisease among people with severe #mentalillness Read it @plosone.org : journals.plos.org/plosone/arti...

Who's seen a case of hypokalaemic periodic paralysis in which they have +ve genetic testing, not just clinical suspicion? #MedSky #NephSky

#NephMadness is merely weeks away Honoured to be a judging member of the Blue Ribbon Panel again this year. This year's panel is studded with current greats of nephrology (and me) ajkdblog.org/2025/02/12/n... @drdeidracrews.bsky.social @ssfarouk.bsky.social
