Big development in this story: Since I first made this letter public, it's been confirmed that at least two other journals have received similar letters. NEJM previously denied to inquiring journalists it had received one. Today, we found out it did, and it has responded to it.

One thing about Chris Van Hollen many might not know: he came up around these kinds of foreign endeavors. His father was a career FSO and US Ambassador, and his mother was a top analyst at the CIA and State Department. Van Hollen was born in Pakistan and grew up abroad.
