Nope. That looks like ecchymosis from an IV run through an IV line on the back of his hand that was removed in the last 24-48 hours. It’s hasn’t started turning yellow-green and dappled on the perimeter of the bruise.

If this bruise is from shaking hands, it is an injury never before seen in any ER or clinic or physician's office.
More likely explanation: someone blew through a vein while setting up an IV. What was the IV for? Anyone's guess.
