DNR doesn’t mean you can’t have healthcare, cancer treatment, etc. elderly is a highly subjective term also, it’s not a number. Also, I find if you talk to people before they’re in the middle of a medical crisis they often make the medically appropriate choices when it comes to advanced directives.

I think people see advocating for a DNR status as "oh just let them die" when in reality a DNR only applies if you suffer a cardiopulmonary arrest. Just because someone is a DNR does not mean they should not get all necessary care up to that point.

I don't advocate for anyone to have a DNR or to be a full code I do advocate that people think about it, read about end of life decisions, and discuss it with their loved & MD

My issue is with the widely held perception that having an opinion favoring DNR is somehow morally inferior to favoring full code.

Legit, intense nuances behind the “DNR: y or n?” question: A friend’s final 60 hours of life were at hospital, after months at nonprofit community assisted living. AL staff told me: “If she’d had a DNR, she could have died here, with hospice care. We adored her & wanted to look after her.” (1/3)

Their pressure to consider DNR had seemed gentle. Death of this strong-willed elder from Stage 4 🫁, 🦴 & liver cancer was a given from the start of the assisted-living stint. The hospital administered O2 (unsure if AL has capability?) and nurses were wonderful - as were assisted living nurses. (2/3)

I just learned from a British cardiologist on YouTube that that's what they do in the UK with elderly and disabled people. Apparently they even do it to them when they're taken to the emergency department, they don't have to be in a nursing home. They just have to be elderly or disabled.

In the UK CPR is a medical procedure & it is a medical decision as to whether it is offered. Futile treatments do not have to be offered to patients. This decision can be made in hospital or the community. Patients are entitled to a second opinion but they cannot demand CPR if it would be futile.

Decisions are made on the basis of whether resuscitating someone would be successful - a medical decision. As doctors, we do not have to offer treatment that we believe is futile. Patients are entitled to a second opinion and to seek legal advice but are not entitled to demand a futile treatment.

No, again, that’s not the case. It’s not unknown to push patients to sign DNACPRs if they’re nearing end of life, but it’s generally considered unethical and strongly discouraged.

Witnessed that it can even be a challenge to have an NDR or a “comfort only” treatment request by a patient respected by HCW.
Also witnessed extreme measures taken on the request of a patient when there was no longer any hope of those measures buying more than a few weeks. In Canada.

To be fair this did sort of happen under Covid in the UK, in that some patients in nursing homes had DNACPRs sent to them to sign in case they got Covid, but they were generally quickly withdrawn when it was noticed.
