if i use my boss or any of my coworkers as a reference applying anywhere else they'd probably say "great for the first few years but then became increasingly reclusive and became increasingly unproductive to the point of just not doing anything"

This is exactly what short term disability is for. You're struggling with burnout, if you can find a doctor or therapist or someone along those lines to back you up, you can take a few weeks or months off to recover. You'll get paid, they can't fire you, and they can't give your job to someone else.
You can also use short term disability to adjust your schedule,like working 3 days a week, or working half days. If your job doesn't provide short term disability benefits, you are at least entitled to FMLA, which is unpaid but something you are legally entitled to.
I really hope this isn't unwelcome, I was looking at someone's profile and they'd reposted something from you, so I went to look at your profile and saw this. I've been through the same thing, and seriously. Short term disability was a godsend for me. I'm sorry if this is intrusive.
