Alternative graders! I'm working on a blog post about "rookie mistakes" -- the things that new alt. graders most commonly do that cause them trouble.
My list so far:
- Unlimited reassessments
- Too many standards
- Super-duper complex system
What would you add?

Rubrics… still tweaking.
Now I bring it up with students most weeks of the semester, at least to some degree.
unbound reassessment = student can reassess w/o any constraints or instructor directives. (whereas bound example = student must meet w/instructor, complete additional exercises, etc.)
Does this help?
That would be different from something like "reflect on what you learned by completing this revision", for example.
(I think I was assuming this was in the context of reassessments, but I think you mean at any time?)
I do think a fairly neutral question like "How did this grading system affect your learning?" would be OK on anonymous end-of-semester evals. The keys are: anonymous and neutral.
