It's mostly visceral, and I think I'm being reductive and using broad-strokes. Generally speaking, it's the overall songwriting approach and approaches to chord progressions and riffing styles. Similarly linked to trad metal as to how power metal is
I personally feel it's both (if we're examining the genre as a whole) but I can definitely agree with this if we're talking about traditional thrash like early Metallica or Megadeth.

This is such a good question. My feeling is extreme trad since early black and death metal kind of went back to punk for some of their cues when they superseded thrash as the zeitgeisty genres
For me they overall embody the bay area sound more consistently and squarely on the nose than any other band. Maybe some albums by other bands (Metallica) are more iconic, but Testicle has done the thing by the straightest definition for the longest amount of time IMO
I think if "the tamest form of extreme metal" is assumed to be a thing, it won't be a genre, it will be a specific band or type of band. If I'm thinking of "tame" as simply safe and with some degree of mass appeal, thrash most often doesn't fit that to me.
Also, I've been listening to extreme metal for 20+yrs at this point, my idea of "tame" is hard to put alongside someone who never has been exposed to extreme music.
isnt all metal extreme to someone, on a philosophical level its about ones perception, someone new may see trad metal as extreme, a veteran may have no extreme, but what happens when extreme becomes normal? does the chicken or the egg come first...

Thrash has overlap w/ the same punk and hardcore influence
But early thrash has some bands that more melodic (thrash/speed) and some that are heavier with harsh vox
The latter is extreme metal the former isnt
