Third. Find and plug the whole, discuss the implications with the team and build processes to fix this and not allow it to become a thing again. Ideally share those learnings. Perhaps a human sacrifice needs to be made also. Either way it will impact schedules as you're now pivoting from a plan.

WELL. First thing first: PR meeting. This is news, how do you counter it? Is there a way to turn this around or at least damage limitation. Secondly internal audit. WHAT ELSE IS THERE? If there's a gap in your process here one can assume something else snuck through. Search and destroy.
