States and private insurers definitely have room! I personally would suspect that MMR remains widely covered (in part for this reason; in part because Kennedy has half-heartedly supported it) -- but not sure about many others!

Government insurers make decisions based on ACIP recommendations; private insurers, in theory, could go beyond it to cover those not recommended! But, whether they *want* to cover more services is the (billion?) dollar question...

Very fair points on all counts! I think socially mediated "second hand" news is a big part of this. And I totally buy that some negative impacts are more immediate and visible (job loss, difficulty accessing soc. security payments), while others will take time.
