We do have screening for HBV in pregnancy but just because it is recommended does not mean it is foolproof or done in all cases. Most importantly, the risk of HBV infection if infected in infancy is drastically higher because there is a >90% chance of it progressing into chronic HBV...

HBV is also not solely transmitted through sexual contact or injection drug use, and there can be more casual activities by which it can spread (such as bites, sharing toothbrushes, razors, etc.).
