Scathing, and absolutely hilarious talk on whether high flow humidified nasal oxygen has a routine role in obstetric anaesthesia. "When you read about airway catastrophes, you don't often think - if only they had a high flow oxygen device". #OAA25ASM

An excellent slide in the next talk, on high neuraxial block. How high does a spinal block typically go in obs theatre. The risk of a higher than intended block is not small, but the risk of respiratory compromise is lower. Both height AND density are required for phrenic nerve palsy. #OAA25ASM

Excellent talk from Dr Heather Nixon on pain during caesarean delivery (PDCD): Review your epidurals. Replace them if they aren't working, and carefully select those you might use for theatre. Believe people. "Pressure" can be painful. How keen are you to put your hand in a vice? #OAA25ASM

Continuing #OAA25ASM is @yavorrm.bsky.social, on forthcoming guidance for anaesthetic implications of chiari malformation. We have covered frequency (around 1%); subtypes, including 1-4, AND 0, 0.5, and 1.5 (!); and why the guideline needs to exist, potentially widening access to analgesia.

A reminder from the always excellent Dr Nuala Lucas, president of the OAA, of just how busy we have become as a subspecialty. The NAP7 activity survey demonstrated that 70% of anaesthesia conducted out of hours is obstetric. What we do as obstetric anaesthetists has a huge impact. #OAA25ASM

The final question in the first session at #OAA25ASM relates to sustainability vs choice when it comes to analgesia, particularly nitrous oxide. Denying analgesic options to people giving birth, to save the planet, is only uncontroversial if it isn't YOUR uterus or perineum we are talking about.

Continuing the updates section of #OAA25ASM, we are talking about inequalities in access to neuraxial analgesia and anaesthesia. Bias, and prejudice, are important contributers to this outcome. Greater diversity in those delivering care potentially provides greater equality in that care.

Next at #OAA25ASM "updates", we are onto PCA remifentanil. Some useful statistics: Around 1 in 4 people receiving PCA remi will have an apnoeic episode in the first hour. Desaturation below 94% also occurs in roughly 1 in 4 people.

The first session of #OAA25ASM is "recent advances" in Obstetric Anaesthesia. Most recent publications on Nitrous Oxide relate to the environment. Healthcare accounts for ~5% of all the carbon footprint of humans. 5% of that is anaesthesia related, and 75% of that is nitrous oxide. #OAA25ASM

Before the conference opens, a representative from Visit Belfast talks about the city. Titanic, tourism, world renowned universities, sunshine (today at least!) and the craic. My mum grew up here, but I've seen more of the inside of the ICC than the rest of the city. I must change that. #OAA25ASM

My son has sent me a petition to sign, about allowing shorts instead of just trousers at his school. I care not at all about the shorts. However, I care a lot about teaching him the importance of protest, collective action, and believing change can result from these things. I'd sign it twice!

Really looking forward to this opportunity to engage with politicians and others over the contributions and needs of the SAS and LED workforce in the NHS. We often say that we are marginalised. Well, we ought to do something about that.

Today's performative clampdown on migrants will gut our care homes of desperately needed staff. We are *already* short of over 150k carers. Banning foreign care workers will only exacerbate this - increasing suffering of society's most vulnerable members & pressures on the NHS. It's madness.

I consider many of the folks who travelled to join this "island of strangers" close friends, actually.

Feeling a little glum today, so my wife suggested I went and pointed a camera in the direction of the sunset. I love that woman. Being reminded that the world is still beautiful is sometimes exactly what I need.
