It's our federal domestic intelligence service. They hunt foreign spies and keep an eye on extremist groups. They are usually a little lenient towards right-wing activity and rather crack down on the left, which makes this assessment all the more relevant.

Additionally, civil servants should not join the party from now on, as joining a party that is officially a danger to our basic law is contra to the legal expectations of civil servants (they may not act against the state). Those that are already members might lose jobs, potentially.

One result: the press may now call them far-right in articles without fearing legal repercussions for slander. It's also a piece of evidence for a legal process to ban the AfD, which would result in it being dissolved + its members never allowed to form another party / hold another official office.

Germany now officially has a far-right party in the Bundestag. The ruling doesn’t eject the AfD from parliament, but it makes any cooperation with them politically radioactive. It also gives Germany's intelligence services the right to conduct more intense monitoring of its activities and members.

If anything, it's a bit surprising that the designation took this long considering that parties and organizations in Germany have independently long treated the AfD as an anti-constitutional party.

AfD has only become more radical since it was first founded in 2013, losing "moderate" leaders such as Bernd Lucke, Frauke Petry and Jorg Meuthen who disagreed with the ethno-nationalist wing. Ironically, it's the party's xenophobic and ethnonationalist rhetoric that led to today's classification.

Alice Weidel, the current leader (known internationally for being best friends with Elon Musk), launched the party's most radical positions such as "re-migration" for refugees, calling Hitler a communist and using lightly-modified Nazi slogans in her public statements.
