In #PalliativeCare clinic, one way that helps me recall patient names (& stories) when discussing cases is to frequently refer to not just the patient but also the caregiver. So instead of "Bob" it is "Bob and Cynthia." Since caregivers are frequently at visits or called after, this helps me a lot.
