Worse still, I’ve seen this used where the “precedent” clearly doesn’t hold either because 1. previous authors used this shortcut in a different situation and or clearly justified (unlike the present author) or where in fact the field has moved on and improved its practice.

This drives me crazy! One result of treating "everyone does it" as an acceptable answer is that, after a certain number of years, few people have any idea which assumptions are based on evidence and which are just conventions.
