Therein lies the problem:
‘Private Insurance’ & ‘Federal Limitations’
100% has to be the only acceptable solution for 100% of the time.
I’m not speaking of full Keynesianism, but a national not a federal model is a better solution.
Not sure if there are politicians brave enough to try 🙁

If the enhanced premium tax credits were made permanent and the 10 hold-out states actually expanded Medicaid we'd be very close to covering everyone in the country who isn't undocumented. But there is a lot of "underinsurance." People stuck with crummy plans that don't protect them financially.

If all states completely dropped their expansions in response (unlikely) we would see over 20 million people lose their insurance. The more likely scenario is a mix of responses and something like 10 million loses. Many would become uninsurable by falling into the Medicaid gap.

Medicaid gap= you don't have categorical eligibility for Medicaid and your income is too high for pre-ACA income eligibility for Medicaid in your state (median is 35% federal poverty line) but you are below 100% federal poverty line so are ineligible for the ACA marketplace.

Why do you need to be at 100% of the FPL to access the ACA marketplace? Because the law was written assuming if you were below that you'd be covered by the Medicaid expansion. (Thanks Supreme Court!)
