Admittedly this was a single isolate from a patient with chronic infection following long term penicillin exposure.

And the fact it wasn't isolated again, hints at a significant fitness loss with these mutations that render bacteria penicillin resistant.

I think the decades penicillin has been around without there being more than case reports of resistance is pretty strong evidence that resistance isn't free.

The amount of penicillin that has been used and failure of PBP mutations to become fixed in the bacterial population suggests to me there is a very significant fitness cost.
I'm particularly thinking about First Nations communities where a lot of GAS transmission, IM BPG, other B-lactams.
