Streeting may well aspire to “bring back the family doctor” (which would be amazing: I had the same GP throughout my school years) and “shift care out of hospital and into the community”: but will his private healthcare donors let him?

Nor does NHS England have any idea whether one of it's mainstay GP patient records system is designed for the job, or cobbled together from software designed for another purose- according to a somewhat delayed answer to an FOI. No wonder they took ages to answer - 'we don't know'

They’ll know one day. It’s happening when a commissioned service at £300 per pt can’t deal with a problem that falls outside their box. Goes to GP. And we’re all learning how and when to say no

In Japan there are no GP's, only specialists paid by government health insurance. Access to healthcare is good and treatment is generally clinically sensible and proportionate. The NHS model can work well, but that doesn't mean alternative models can't also work well.

Different models with UK a system that has been developed around having specialist generalists who screen, manage and prevent specialists having to deal with 90% of what might come their way.
The Japanese system relies on hospitals that have high occupancy & much more developed tech than UK
