My assumption in writing the books is that addiction is more than just the biochemical problem. It’s literally a habit and coping mechanism, so typically someone in active addiction will go back to the habit even if the structure changes. They choose.
And also, Adam knows all the tricks. Anything they can do to him he can undo, as long as his Ability still works.
And the Guild considers it highly unethical and wasteful to burn out an Ability as anything but a last resort. They will threaten, but they are scared if they do it…
Given that Satin targets the Abled part of the brain as well, it may also have changed things in ways that would make traditional structure therapies not work well.
