yeah, elden ring was pretty messy on that front. It tried to use its usual formula of scattering obscure steps and encounter around so the quests is satisfying to piece together (and some are going to be missed) but the open world format just isnt compatible with that approach. Too much room to miss
They developped a few interesting techniques, like varre and a merchant leaving a message to tell you where to find them, and dung eater speaking to you as a phantom in the hold, but those were vastly underutilized.
Npcs like Alexander and Millicent had some super obtuse spots to find.
I will say through that the dlc did a much better job at guiding you toward its npc quests. I ended up completing most of the main ones without having to look things up.
I think it was the to the DLC advantage that it has quite a few funnels and landmarks to place those npcs on.
