I do not fully agree with this kind of algorithms because make physicians like robots. The microbiological part should not be applied to all patients with suspected meningitis. If I have patient with purulent CSF, I will not send samples for viral causes, and vice versa (in most cases).

If we had guidelines that encompassed the true complexity of meningitis... No one would read them. So I'd rather simple guideline followed than complex ignored

What do you mean? We do LPs on daily basis in emergenc department, we always have basic citology and biochemistry done in 30 minutes after LP.

Patient with few days of classical symptoms and signs of meningitis and clear CSF with lymphocytic pleocitosis is highly unlikely to have bacterial cause.
