Go-arounds are not that rare. For separation or weather, including fog. I've personally been through a handful of them. They are not investigated unless there are more serious issues

A departing aircraft holding longer than expected to ensure they're ready to takeoff is a common enough reason to go around. Midway was a near miss, this was a routine go around to accommodate a takeoff that delayed a little too long in starting their roll at a busy airport with tight spacing.
