On the subject of the last one standing between the party and a TPK, I've been considering a homebrew rule to give parties in the death spiral a bit more of a fighting chance, but I'm sure it's got balance issues out the wazoo.
Basically the idea is that the surviving party members can assume the downed ally's initiative space until either combat is over or the ally gets revived. Hopefully the fact that the players would be expending more personal resources as they take a second, third, fourth turn will balance things out.
What if they could roll initiative again, with a +1 for every downed party member?
Or... they get a second turn on one party member of their choices initiative? This only works if they're the last man standing.
