Lots of people better situated than I am to comment on what went wrong, but re: the information environment, I do have some thoughts from here in rural Ohio.
Actual news media coverage does matter, but most of the reachable folks here get much of their info from social media.

And also, the entire last four years has been an endless fountain of viral bullshit flooding all of our feeds out here.
It is almost always first person, non election content, at least on the surface. It's a regular person "sharing their experience."
A fact check in the news media is ok, but most don't read it or share it.
I don't have good answers for what does help, though. What I do know is that "the truth will out" is not a plan.
