I tell my medical students to eliminate the phrase "poor historian" from their vocabulary. It's a bad phrase that makes it easy to ignore what the patient says as soon as they say something that doesn't exactly fit what you've previously read in a textbook.

Do me a favor if you want to get into medicine but only take care of perfect patients who know their medical history and all of their medications and get upset when they don't, maybe think about why you want to be a doctor.
