She said they considered doing it in Los Angeles, but the bureaucracy and and risk was too high. She flies down for a week a month to help people where she can. It was too hard to help people in her own country.
They can't afford to practice medicine with malpractice insurance rates on one side & insurance companies nickel and diming them on the other side, it's not sustainable.
