My wife asked the same question. My guess is they've observed the historical footage of one individual to guage their characteristics with current data and studies. Then, they observed the behavior of the other person. There is probably a base line to work with, and they went from there. It's doable

Yeah, but at what point did they evaluate? He was much more erratic and emboldened in the later years than the early years. So did they pull from a specific period or his entire reign? It's a question of what behavior earned the score.

It doesn't matter when you cross the finish line. Behavior not seen beforehand might have been hidden. We have mental illness in our family from our maternal grandfather. He was never diagnosed as mentally ill, but he was. My mother hid it very well into her 80s.

I'm wondering what other interesting questions about brain function might be uncovered by analyzing perceived mental state as someone progresses down the path of unfettered power.
