I think we should add local building and fire codes in the equation, and thus firefighters and city staff being particularly picky because ultimately this is not their money, but the agency's ones.
That's at least what I get from documents and early hints talking with people.
This mostly doesn't apply to me, because the MTA isn't subject to city rules (being a state entity). But once we touch the street or utilities, DOT/DEP/ConEd get involved and boy do things get complicated. I'm sure local gov is bad elsewhere
Let's just say that I definitely think we need a culture change in project delivery. And it has to start from the top - rewarding and promoting people who are willing to do things in a less risk-averse way
Honestly, no-one is losing their job in any case at most of these agencies - but to get promoted in a code department, you're probably going to want a reputation of being really strict about code
