Group of us from work had a meal out, everyone paid for their own. The boss lifted the receipt and later claimed for everyone's meal on his expenses. Wasn't sacked but his credibility took a hit.

Where I used to work (Atkins) we'd see small uncontroversial projects with documentation too bulky to ever print out: environmental assessment, wildlife surveys , CEEQUAL, BREEAM, we'd a whole department churning out or "implementing" the stuff.
