So I use an 18G SonoTap Tuohy from Pajunk.
In an ideal world I would use a 20g version of the same - but we just stuck with the first needle we got and it works pretty well.
I have asked for an NR-Fit version of this… have yet to see it!

I am pretty sure @jeffgadsden.bsky.social is using up to 5mL per nerve, but he can clarify.
I am using up to 2mL maximum

TSA can either be used to achieve segmental spinal anaesthesia per se OR lower thoracic approach can be used for lower limb anaesthesia when a fused/scarred lumbar spine demands it ..a single shot avoiding multiple failures lower down👍

So..we all do thoracic epidurals !!
Would you rather have a carefully considered and delivered thoracic spinal puncture performed by an experienced anaesthetist with a 27g needle OR a 16g thoracic epidural placed by a trainee??…just a thought 🧐

Ok guys ..listened to the first half last evening, then it was sleepy time. So I switched off to enjoy the second half on the way to work today…my wife complained!! “ I was enjoying that” says she!!
I think since she’s now met Amit in the flesh she can relate..she loves it😘
