UTIs can be caused by vigorous sex in some women, I've read: not necessarily a hygiene deficiency in them or their partners.
Is she not getting UTIs now because she's not having (as much) sex, perhaps?

I got the impression that he's never asked for, and she's never given, consent for violence. They seem to pickup on a vibe and agree later that the violence was enjoyable.

Are there old age homes for dogs? That's likely where he'll put her if she goes senile before he does. I'm sure she'll do the same to him using his treatment of this old dog as her excuse.
