Had 2 ED colleagues stand up and say that in the last month, while letting patients know about unexpected pregnancies, the patient asked if it was safe to talk about their options with the ED doc, because they were afraid of the consequences of saying that they were considering abortion.

One time had an ICU nurse how much money it would take me to take a slurp of the respiratory secretions from a suction canister and I’ve never been able to look at them the same since. Rectal exams, no problem. A bit of blood on my wrist from examining a trauma patient, no big deal. Sputum? *hurls

I’ve started getting requests for them from scribes in our ER for med school. I usually ask them to tell me what qualities they would like me to emphasize and ask if they can think of some examples of situations that highlight that. Makes it much easier than digging through a resume

Nothing has topped the magic of those for me. Day/Night cycle, breeding, Shinies, backward compatibility, etc. Sitting in my parents basement with my brother and a link cable playing those games for the first time is peak childhood.

Not a fan. Setting aside the fact that you do everything possible to not intubate DKAers, bicarb breaks down into H2O and CO2, so really should only be given when you have a way to blow off excess CO2. Also, giving hypertonic fluids to someone already hyperosmotic seems bad?

Do you happen to know if the distribution changes in patients who get regular paracentesis? I’m kind of surprised the staph rate is so high. I usually just do cephalosporins and wondering if I should start covering more broadly.

I get this. I really do. In my mind, the respect that Biden has showed for peaceful transition of power is the real message being conveyed: doing the right thing and showing REAL leadership is the best revenge. Trump can never hope to be this presidential.
