It had to be said and strong words AND actions are required “the ambition to move care from hospitals has stalled.NHSE must ensure more funding in community in line with its own ambitions”

GP deserts in Australia. Again those in areas of higher poverty miss out. We need to act now to stop this in England as we are also heading towards GP deserts

Important points in this report- design and planning and ongoing supervision and support are key ingredients if we are to have a successful staff mix. The ARRS in GP roles came with the carrot that they were “paid for” but the PCNs successfully implementing them took time to train & supervise them

Crucial point here that additions to the waiting list are lower than before.GPs are using advice and guidance & also using our gatekeeper role to ensure referrals that are made are necessary. We must ensure that people who need procedures such as operations or specialist treatment, still get them

Brilliant from @becksfisher.bsky.social “Advice and guidance can be effective if used judiciously for the right patients and the advice back is timely and of high quality” We have been using A&G for years in NEL - it’s not a wand to reduce waiting lists- it’s appropriate for when advice is needed

Lots of plans being produced by government- the elective care plan, the 10 year plan, the plan for change- will we the public have the patience to see the action and results when the current situation for patients whether in hospital or under GP care is frankly dire

Tough decisions for the government to make: investing in the NHS is essential but we also need to make longer term gains in public health and tackle population health and poverty

This is spot on again from insightful @alisonleary1.bsky.social If workforce planning is reduced to counting numbers, we forget that new starters & new roles (eg the multitude of ARRS roles in GP) need supervisory support. We need to value our experienced workers & try to retain as well as recruit

Our GP clinics are full of people suffering from winter viruses & hospital admissions for flu are up 3X more than last year. It’s never too late to have your winter vaccinations to protect yourself. Book in now at your GP practice or pharmacy

The Carr Hill formula for general practice funding negatively impacts practices in deprived areas the most reinforcing the inverse care law. Changing the formula is necessary but not easy- we need a needs based or activity based approach. The briefing from @nuffieldtrust.bsky.social poses some ideas

The operational processes mentioned in point 3 in the Health Foundation analysis is one to focus on especially at the primary/ secondary care interface- doesn’t need investment-needs joint planning-seeing so many children attending ED, contacting 111 & their GP with same virus-needs coordination

Great reminder and one to remember in busy general practice- let’s be curious not furious! Mistakes happen, what can we do to learn from them and prevent it next time

“It’s important we don’t reduce moving care closer to home to a grab bag of separate shiny new initiatives” It won’t necessarily lead to cost savings (the 5 year forward view didn’t) We need to be clear what we mean by “care closer to home” Use basic QI and be clear on what we are trying to achieve!

Got my certificate to be an Actor Learning Set facilitator! Was a great 6 months of learning & practical experience in facilitation.I do hope ICSs & senior leadership in the NHS will adopt the Action Learning Set Approach more widely to help people solve the tricky issues through shared reflection

A mixture of a social care insurance model with a modest top up from the individual seems to be working in Japan where 1 in 3 people are aged over 65. What can we do in the UK to improve social care which is on its knees?

Delighted to have been invited to beautiful Kuwait to speak at their 6th Annual Primary Care Conference hosted by the Ministry of Health. A great opportunity to share learning on an international scale and enjoy some winter sun. Alhamdulillah

@becksfisher.bsky.social highlights the gaps in the PM pledge, reducing health disparities between the rich and poor and social care. Reducing hospital clinic wait times is of course necessary, but will not serve the government plans of prevention, shifting care into the community and healthy lives

Would be interesting to get a breakdown of which healthcare professionals the appointments were delivered by (not sure if this data is available) I am assuming it was not just GPs - in any case this is good news for patients who are struggling to get access especially with BMA collective action

Looking forward to attending this event which is so important right now with the governments pledge. How do we move care out of hospital and closer to home and what are the pitfalls to look out for?

New weight loss drug is recommended for people with bmi of more than 35 and at least one weight-related illness ( about 3.4million in England). But only around a quarter of a million people living with obesity to be offered the weight loss drug over next three years. www.nice.org.uk/news/article...
