My new idea for cutting down on 3 point attempts in the modern NBA is no fouls called on 3 pointers. I'm talking middle linebacker blasting a QB like it's 1985 kind of action beyond the arc. Make White think twice about taking a trash shot like that.

I don't worry so much about it in my peds CVICU, but in absence of RV dysfunction/pulm HTN consider pulm vasodilators for V/Q matching purposes to be more of a salvage therapy attempt. Regardless would pay close attention to volume status, LV afterload and follow MVO2/NIRs to follow LV.

The hypothesis of vasodilator only effects of milrinone in the pediatric post-cardiac surgery population also would struggle to explain the benefits in mitigation of Low Cardiac Output Syndrome found in the PRIMACORP study in a biventricular repair patient cohort. www.ahajournals.org/doi/10.1161/...

Will have to see if I can access the full article, but one possible issue other than the small number of enrolled patients is the timing of their post-milrinone initiation check at 4-6 hours. This is in line with a post-bypass "slump" timeframe that could give us a worst case interpretation.
