We eventually figured out that this was a new entity, “xanthogranulomatous epithelial tumor”. Locally aggressive, non-metastasizing. They turn out to have unique HMGA2::NCOR fusions. “Keratin-positive giant cell tumor of bone” is the same thing. pubmed.ncbi.nlm.nih.gov/38278607/

It contains an EWSR1::CREM fusion and corresponds almost perfectly to Case 6 from this excellent Japanese paper. So, perhaps there is a yet to be fully discovered, SEF-like sarcoma with this fusion? Stay tuned, I guess. pubmed.ncbi.nlm.nih.gov/31305268/

One final comment. It is interesting that SRF is involved, as it is also involved in some distinctive myoid tumors, often seen in children. I’ll come back to this subject one of these days, after final acceptance of a paper. This is a nice paper on this subject. pubmed.ncbi.nlm.nih.gov/28248815/

It is nicely positive for SMA, caldesmon and desmin (not shown). But- it is also positive for both MyoD1 and myogenin. NGS found SRF::NCOA2. Leiomyomatous spindle cell RMS. The NCOA2 fused ones usually have a good prognosis. C. Dehner, now at IU, wrote a nice paper pubmed.ncbi.nlm.nih.gov/37422156/

This is a tale of two clones. A lot of labs use the E89 caldesmon clone, which is totally non-specific. See how it has a tram track, myofibroblastuc pattern? That’s not caldesmon. You want to use the highly specific H-CD clone. pubmed.ncbi.nlm.nih.gov/29687929/
